*state requires a video visit with the doctor to establish care.
 

You are already on a Brand Name GLP-1 medication. 

or

 You are on a Compounded GLP-1 medication and looking to transition to a Brand Name GLP-1 Medication.

or

You were on a GLP-1 Medication in the past and want to resume care.
 

Right now we really want to work with folks who are already on a GLP-1 medication and simply looking to establish care with a program that cares about them as individuals. Of course we can now also accepting members who are Brand New to GLP-1s, but this gets tricky. Our insurance team has been doing an amazing job submitting accurate and fast PAs and helping folks secure coverage, but it's important to remember that we have no control over your benefits.  Some plans may require you use certain providers too, so please check call and check your insurance coverage benefits or restrictions, copay expectations and medical requirements for coverage before joining!  Cash pay is definitely still an option. We can send your RX to Lilly Direct and or help you navigate the Manufacturer coupons! If this works for your budget, we'd love to be part of your care!
 

You understand that the cost of the program is $150 per month and this is due at the time of sign up. There are no additional fees. The cost of your medications at the pharmacy are not included. While we are able to complete Prior Authorizations as needed we cannot guarantee these will be successful. Please check your insurance coverage and plan requirements prior to joining.
 

To make your intake process as smooth as possible, please have these items ready  in the device (computer or phone) that you will be using before you start...

• Your Medical Insurance Information Including Photos of the Front and Back of your Insurance Card. Bonus if you have a PBM (Pharmacy Benefits Manager) we need that information as well. 
• A photo of your driver’s license or passport.
• A recent "Selfie" photograph that matches you to your ID. 
• A picture of your current GLP-1 medications with the pharmacy label containing your name OR the pharmacy or insurance records that show you have filled your prescriptions.
• Any medical records you may have related to your weight management history and/or any documentation you can provide that show your weight history and trends. PDF or screenshots work!
• A copy of any lab results that you may have from the past 12 months. (optional, but encouraged).
• Your usual blood pressure and resting heart rate.
• Your preferred pharmacy information (name, store number, address and phone number)!
  

It is very important that once you start your intake forms you see it through to the end. It  should only take about 10 minutes if you have your documents easily accessible.  If you made it past the payment screen, and have to stop the intake forms for any reason, please DO NOT start over from the beginning of the eligibility quiz (do not pay more than once!).
